What is Chronic Care Management?

Chronic Care Management (CCM) offers personal support to patients with complex needs. This added level of care and service leads patients to a healthier lifestyle by proactively managing their care and providing a trusted clinical resource they can reach around-the-clock.

CCM is designed for patients with two or more chronic conditions. Patients enrolled in CCM services benefit from an entire care team focused on their needs, increased communication with their provider and improved care coordination.

2 Morrow Health can help identify qualified patients in your Electronic Medical Records (EMR) and contacts them for enrollment in a patient-centric process.

What does Medicare pay for CCM?

  • CCM first 20 mins each month $62.35* (recurring)
  • CCM each additional 20 mins each month $47.10* (as needed)
  • CCM Care Plan set by physician $64.25* (one-time)
  • Years 1 Reimbursement of $1000+ per patient
  • 100 Patients = $100,000+ per year
  • 250 Patients = $250,000+ per year
  • 500 Patients = $500,000+ per year
  • 1000 = 100,000 per year

What is remote patients monitoring for Medicare

We provide medical devices to the patients (blood pressure, blood gl glucose da form)
These devices are connected to the patient’s phone. After each reading on the device, the results are automatically sent to our team. Any out-of-range readings will prompt a phone call from our care coordinator.
